Virginia Delivery System Transformation

Virginia submitted an application for a new demonstration program that would provide enhanced services and coordination through a Medicaid managed long term services and supports program. The demonstration proposal aims to improve how care is provided, coordinated, and financed for high-risk and high- utilizing beneficiaries through undergoing delivery system reform. The federal public comment period will be open from January 29, 2016 through February 28, 2016.
